For 2017, a survey was developed by AAO-HNS and AOA staff members, based on member and committee feedback. After the survey form was finalized, invitations and reminders were sent during the period of June 12–July 21, 2017. There was extensive follow-up with non-respondents to encourage participation and individual follow-up with respondents who reported data that required additional confirmation to ensure accuracy of the final report. These charts are excerpted from the full report.
